Renocardiac Neurohumoral Axis
1974 - 1988
The late 1970s through the 1980s saw the renin-angiotensin-aldosterone system consolidating as the central organizing framework for cardiorenal disease. Angiotensin blockade via angiotensin-converting enzyme inhibitors revealed favorable hemodynamic effects and improved renal function across congestive heart failure presentations, anchoring cardio-renal management in neurohumoral control. At the same time, dialysis emerged as a distinct cardiac risk substrate, with dialysis-related cardiovascular pathophysiology and imaging patterns highlighting accelerated atherosclerosis, dialysis-induced hemodynamic shifts, and the cardiorenal interactions shaping cardiac function. Vasodilator and diuretic strategies clarified renal hemodynamic responses and neurohumoral consequences in heart failure, and invasive assessments reinforced the role of renal failure in driving cardiac function alterations.
• RAAS-centric frameworks emerged as a unifying paradigm for cardiorenal disease, with ACE inhibitors and angiotensin blockade revealing hemodynamic benefits and renal function modulation across congestive heart failure presentations [2], [4], [5], [8], [10], [12], [15].
• Dialysis-associated cardiovascular pathophysiology and imaging/monitoring patterns reveal accelerated atherosclerosis, dialysis-induced hemodynamic shifts, and cardiac function alterations measured by echocardiography and invasive techniques, underscoring dialysis as a cardiac risk substrate [1], [7], [9], [17], [16], [18].
• Vasodilator and diuretic pharmacotherapy patterns highlight renal hemodynamic responses and potential renal function consequences in heart failure, captured in nitroprusside-based strategies, hydralazine interactions, and diuretic neurohumoral effects [3], [6], [14].
• Imaging and invasive hemodynamic assessments illuminate how renal failure shapes cardiac function, from cardiac output shifts to diastolic function, shaping the understanding of cardiorenal interactions [7], [9], [12], [17].
